HRT replaces declining estrogen, and when the uterus is present, includes a progestogen to protect the uterine lining. It is most helpful for healthy women with moderate to severe menopausal symptoms, especially if started before age 60 or within 10 years of the final menstrual period. Decisions are individualized based on symptoms, age, time since menopause, personal risks, and preferences.
HRT is the most effective treatment for vasomotor symptoms and is effective for the genitourinary syndrome of menopause. It can also improve sleep and quality of life in symptomatic women. Nine common perimenopause and menopause symptoms include:

You can use systemic therapy for whole-body symptoms or local therapy for GSM.
Systemic estrogen: oral tablets, transdermal patches, gels, sprays.
Progestogen: required if you have a uterus, given as oral micronized progesterone, levonorgestrel IUD, or certain combined patches or pills.
Local vaginal therapy: low-dose estrogen creams, tablets, inserts, or rings for dryness and pain with sex.
Transdermal estrogen has a lower risk of blood clots compared with oral routes and is often preferred for women with clot risk factors. Your clinician will match the formulation and dose to your symptoms and risk profile.
Risks depend on dose, route, duration, and whether a progestogen is used.
Blood clots and stroke: risk is higher with oral estrogen and lower with transdermal routes.
Breast cancer: combined estrogen plus progestogen is associated with a small increased risk that rises with duration. Estrogen alone after hysterectomy shows little to no increase in risk.
Gallbladder disease may be increased with oral therapy.
